Malware doesn't have to be on Daves computer, it could be on any computer where Daves email adress is on. (This is just one possibility, but a probable one). So theres malware harvesting email adresses from the computer and sending emails from that computer. If it only sends 2 per second, this already is 7200/h. With Sender and recipient names maybe chosen randomly from the harvested adresses, who cares, no one should take this personally.
Very easily spoken, due to the smtp protocol, "From", "Reply-To", "To", "Cc" etc are only content of the mail but need not correspond with the real sender and recipients. For instance I can send a Mail as Anton Bruckner, Sender appears as Nietzsche, Recipient appears as Allan Pettersson, but there are five recipients except Allan Pettersson, furthermore, delivery error messages go to Richard Wagner. Fun with SMTP! :D

I'm receiving ~1500 to 8000 Mails per month and couldn't care less if a mail tells I'm the sender or receiver or whatever. BTW I'm routing my mail through spamfence.net - maybe someone remembers the worm "sven" a couple of years ago. Usenet citizens suddenly received tons of spam because of this. From this day on I started filtering with spamfence. Some stuff goes through, but it doesn't produce false positives. I can really recommend it and I know many usenet veterans rely on it. Free for personal use.
