The word has been appropriated for modern slang, along with " dope ", " trippin' ", " fire " etc. Within that context, normal definitions and usage no longer apply. So, no " failure " exists, it's simply a usage you're unfamiliar with.

As much as I think the film version of Quatermass and the Pit is the pinnacle of the horror/scifi subgenre, the televised version from 1958 has Andre Morell, the best actor in the role of Bernard Quatermass.

Morell also played Tiberius in the wonderful series The Caesars.
